OKLAHOMA VENDOR REGISTRATION
TO OBTAIN VENDOR REGISTRATION FORMS VIA THE INTERNET:
1. Visit
the Department of Central Services web site at:
www.dcs.ok.gov
2. On the
home page, click "Central Purchasing" on the gray menu bar appearing at the
left.
3. Then click
"CP Rules, Statutes and Standards" on the gray menu bar.
4. Click on
the hyperlink to "Central Purchasing Rules".
5. After you
have read through the rules, back arrow to either the CP Rules, Statutes
and Standards page or the Central Purchasing home page.
6. Click on
"Vendor Registration Information". There are four vendor documents that you
will
need to print off. They are:
Form 040: Vendor Registration Procedures
Form 041: Vendor Registration Application (can be filled out online)
Form 042: Substitute IRS W-9 (can be filled out online)
Form 043: Commodity Class Listing
NOTE: The Vendor Registration Application will ask for a commodity class. You will probably want to register for Commodity Class 0952, which is for "Human Services", and possibly CommodityClass 0961, which is for "Miscellaneous Professional Services". Other commodity classifications are listed in Form 043. A $25 fee is required for registration of each class and will need to be submitted with the completed Vendor Application and the Substitute IRS W-9. Checks should be made payable to the Department of Central Services.
SUBMISSION OF FORMS:
Forms 041 and 042, along with the fee for commodity class listing(s), may be submitted by mail or hand carried to:

ADDITIONAL INFORMATION:
You will need to obtain a Certificate of Incorporation or a Certificate of Authority prior to the award of any contract. However, these documents do not need to be submitted with the Vendor Application.
A Certificate of Incorporation, Not For Profit, may be obtained by Oklahoma organizations that have registered with the Secretary of State’s office to operate within the state of Oklahoma. If you are not already registered, you will need to do so prior to submitting your request to be placed on the state bid (vendor) list. Oklahoma organizations may register and/or obtain an application for the Certificate of Incorporation by calling the Secretary of State’s office, Filing Division, at 405-521-3912. A $25 fee is required.
A Certificate of Authority (for organizations registered as Not for Profit within another state) may also be obtained from the Secretary of State’s office by calling 405-521-3912. There is a $300 charge to register a foreign corporation within the State of Oklahoma.
